Admissions
Clients needing short-term rehab therapy or long-term care are welcome to apply for admission. Applications are available below or by contacting our Admissions Specialist. We will hold applications as long as you need. All forms of payment are accepted.
We have an expedited process for those in the hospital. You or the hospital discharge planner may contact us. Private rooms are available for Medicare short-term rehabilitation on a first-come basis.
Our hospital liaison will meet with you to review your needs and determine the best location for you within our welcoming community.
